Parking & Building Access

There is a parking lot and street parking available. You will enter the building through the side door off of the parking lot labeled “second floor entrance”- this door requires a code- please text the clinic at 612-516-3184 or email hello@azaleampls.com if you do not have a code.

Cancellation Policy

Out of respect for our micro-business, please only book appointments you intend on keeping. Patients who habitually reschedule or cancel their appointments may be blocked from online booking. Please note that by making an appointment at Azalea, you are agreeing to our cancelation policy: We require 24 hours notice for any cancellations or changes to your appointment. If you do not cancel or reschedule within the 24 hour window or you no show your appointment, you will be charged 100% of the appointment booked. A late cancellation fee will be applied in the event of illness, such as cough, cold, flu, other illness & COVID-19 exposure or diagnosis.
